Job Details:
Title: RN Case Manager
Location: Seattle, WA (On Site)
Type of Engagement: Permanent
Rate: Competitive and Dependent on Experience
Job Responsibilities:
- Management of care for a set patient population in hospital setting
- Follow through with patients to ensure optimum care
- Provide support through detailed assessment, implementation of care for specific patients needs
- Five years if acute care in medical/surgical and/or ICU/CCU setting required
- Certification in Case Management (CCM) a huge plus
- Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) strongly preferred
