PRIMARY RELATIONSHIPS: Interdisciplinary Team, Social Workers, Center Directors, Long Term Care Facilities and Staff, Participants, Participant's families and friends, facility staff.

OBJECTIVE: Under the supervision of the Director of Social Services plans, organizes and implements ministry services to Senior Community Care participants and families. Responsibilities include but are not limited to: assessment, teaching and counseling to participant, caregiver or other appropriate representatives. The Ministry guidance could include individual participant contacts; appropriate collateral contacts; participant and families, assessment and counseling; provision of resources; ongoing case management; support from a palliative and end-of-life care perspective, advocacy to ensure participant and caregiver needs are met and addressed. The Chaplain is the ministerial liaison between the Interdisciplinary Team (IDT), caregiver representatives, and community agencies.

Schedule: Monday-Friday (Occasional Weekends)

**Spiritual care may be required during times that are not during normal business hours, including evenings, early mornings, overnight and during the weekends. PACE does provide EOL care and Spiritual support is an essential component of the EOL program**

Compensation: $47,000-$55,000

  • Bachelor's degree (B.A.) from four (4) year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
  • Working knowledge of current word processing systems, e-mail.
  • Possesses well-developed interpersonal skills.
  • Team oriented and has ability to work effectively with internal and external customers.
  • Good communicator.
  • Must be organized and enter participant records data in a timely manner according to SCC guidelines.
  • Possesses good customer service and conflict resolution skills.


  • ESSENTIAL FUNCTIONS:

  • Functions as a consultative member of the Interdisciplinary Team.
  • Participates in Interdisciplinary Team meetings as needed; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Acts as facilitator for meetings with participant, family and/or caregivers to assist with spiritual issues regarding the aging process, dementia, grief and loss, end of life, plan of care and any other spiritual needs.
  • Prepares and delivers sermons, devotions, memorial services and other talks.
  • Attends and participates in End-of-Life program meetings, visits, and related care.
  • Oversees the End-of-Life bereavement program.
  • Regularly enters progress notes in EMR after conducting participant interactions.
  • Meets with participants to assess spiritual needs and enters that information in the EMR in accordance with SCC guidelines.
  • Works closely with supervisor to update information on tasks and spiritual guidance delivered.
  • Responds to emails and messages within 24 workday hours.
  • Counsels those in spiritual need and comforts bereaved.
  • Is responsible for adherence to program cultural standards (SCC Fundamentals) including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.

    Primarily works in nursing home. Must be able to travel as required by job responsibilities. Works in office areas as well as throughout the facilities. Interacts with staff, consultants and government agencies. May be subject to hostile and/or emotionally upset staff.
