Job Summary:

The Division of Academic Specialists in Obstetrics and Gynecology in the Department of OB/GYN has an opening for a 0.40 part-time University Faculty Clinical Instructor position.

The Pregnancy Medical Home program in the OB-GYN UCHealth practice is a multidisciplinary team within the department of Ob/GYN providing longitudinal medical and social support to pregnant and postpartum moms. This program targets delivery of prenatal care and transition of care after delivery to interpregnancy care for the Medicaid patient population at UCHealth via creation of a Pregnancy Medical Home. This unique care model is expected to both improve the quality of patient care delivered and maternal and newborn health outcomes. In addition to the social worker, the program employs a full-time nutritionist, two full-time nurses., a researcher, two nurse-midwives, and an OB-Gyn. As part of this team, the part-time (0.40 FTE) social worker position will be required to work 2, remote 8-hour days a week (in-person is an option). The Division of Academic Specialists in Obstetrics and Gynecology in the Department of OB/GYN is responsible for this program.

The Social Worker will work within a team setting to assess patient needs at entry into the program. They will provide screening interviews to identify at-risk social and behavioral patients entering the clinic. They will provide education and counseling addressing behavioral and social needs of patients. They will coordinate referrals to resources within the clinic, campus, and community to further address medical, mental health, behavioral health, and social needs of the patient. They will provide education to patients. They will provide outreach and education to community resources about the program. When appropriate, the position will triage to interdepartmental and community resources to provide additional multidisciplinary care.

Key Responsibilities:

When identifying at-risk patients this position must be able to make critical decision regarding the patients' mental health needs and social work assistance. Ability to work within a team setting that includes clinical psychologists and psychiatrists is critical.

Position must be able to identify and create a list of community resources for individuals needing long-term care. Position must be able to assess the patient's needs and provide them with the proper clinic and community referral.

Position will be responsible for developing educational information. This will require the position to use available resources to create training materials, know what education materials are needed and how best to put materials together.

The Social Worker should have strong assessment skills, be able to develop appropriate treatment plans for individual patients, be able to recognize and intervene in emergency situations and provide crisis intervention.

The Social Worker will need to recognize social needs of patients and be proactive in providing support services to at risk patients, notify other staff members of their need for involvement in the patients care. Building a relationship with patients that fosters early communication of risky situations to allow for preventative interventions over crisis management.

Qualifications:

Minimum Qualifications:

Applicants must meet minimum qualifications at the time of hire.

    Postgraduate training and/or state licensure is required in LCSW.

Preferred Qualifications:
    Experience working in care management, care coordination, or patient navigation Experience working with EPIC.

Knowledge, Skills and Abilities:
    The person should have knowledge of human development and psychological theory as it relates to pregnancy, human behavior, and addiction. They should understand the normal pregnancy course and its impact on physical and psychological states and relationships. They should possess relationship building skills across a diverse range of cultural settings. Facilitation through Motivational Interviewing techniques is highly desired. Ability to communicate effectively, both in writing and orally Ability to establish and maintain effective working relationships with employees at all levels throughout the institution Outstanding customer service skills including the ability to supervise Dietary needs for people of diverse backgrounds

Anticipated Pay Range:

The starting salary range (or hiring range) for this position has been established as HIRING RANGE:

$24,400 - $27,200 for a 0.40 of a 1.0 FTE ($61,000-68,000 for a 1.0 FTE)

The above salary range (or hiring range) represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. This position is not eligible for overtime compensation unless it is non-exempt.
