Description:

The Clinical Nurse Leader (CNL) is an expert nurse and role model who facilitates expected outcomes, demonstrates leadership and serves as a resource in the management of clinical nursing care to improve patient outcomes within the defined clinical area of practice and functions to provide holistic, patient-centered care using evidence-based practices.

The CNL is responsible for evaluating the appropriateness of admissions and continued patient stay and communicating patient needs to the care management and inter-professional team to facilitate the development of an appropriate care plan. The CNL contributes to the healthcare team's effectiveness by identifying barriers to effective and efficient utilization of resources and evaluating patients' needs.

The CNL facilitates communication across the inter-professional team and actively participates in interdisciplinary rounds, complex case review and management of patient throughput. The CNL evaluates, monitors, and improves the quality of nursing care by leading/participating in performance improvement activities and providing/coordinating department-level nursing orientation and ongoing education that support quality patient care and achievement of identified outcome goals.

The CNL assists in organizational initiatives as related to ongoing maintenance of specialty designations, regulatory requirements and community collaborative efforts.

The CNL provides department-level oversight for the following: • Improving clinical outcomes, care coordination • Maintain professional practice environment for Nursing • Ongoing education and professional development for clinical nurses • Manage clinical component of orientation to nursing department • Nursing care coordination of complex cases
