Position Summary: Provides skilled nursing care to patients in their home, including nursing assessment, interventions and supervision of Home Health Aides and Personal Care Aides in the field on a per visit basis.

Principal Responsibilities:

  • Conducts new patient clinical assessment and follow up re-assessments and aide supervision according to agency policies and procedures.
  • Establishes and provides a plan of treatment for the patient based on results of the nursing assessment, patient needs and physician orders.
  • Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to the patient's care and safety. Makes referrals to other Agencies and Community Based Services as indicated by the patient's needs and reports actions to the Director of Patient Services.
  • Involves the patient and appropriate others in the planning, implementation and assessment of the intervention/treatment program.
  • Observes patient signs and symptoms and reports to the authorized practitioner and/or other appropriate health professional as often as needed, or upon changes in the patient's condition. Makes medication reconciliations/management, and education as needed.
  • Provides education to patient/family and paraprofessional staff as appropriate, on plan of care, disease process, medication, universal precautions, infection control, nutrition, safety emergency measures, patient care and community resources.
  • Instructs, supervises and evaluates paraprofessional according to agency guidelines and more frequently as the patient's care plan and needs warrants. Ensures that all home visits are performed within the guidelines of agency standards and the New York State Department of Health requirements, Documents services provided, responses to services, changes in status relative to plan of care. Tracks visits and medical orders for assigned patient case load.
  • Communicates changes in patient’s condition, treatments, medications, progress towards goals and response to treatment on a weekly basis to the Clinical Manager of DPS.
  • Plans for discharge of the patient taking into consideration goal achievement and provides for follow-up or referral.
  • Performs other duties as needed or requested.


  • Job Competencies & Minimum Qualifications:
  • Graduate of an accredited school of nursing; Bachelor’s degree (B.A.) in Nursing from a four-year college or university One year of hands on nursing experience.
  • Must have current license and registration to practice as a Registered Professional Nurse in New York State. One year Home Care experience preferred.
  • Problem solving abilities while mutli-tasking; ability to exercise sound judgement
  • Excellent customer service and organizational skills
  • Ability to ensure compliance with all regulatory and company policies regarding patients, caregiver and general practices.
  • Ability to work on own initiative
  • Must be able to travel locally to make home visits. Valid driver’s license, car registration and insurance as applicable.


  • Working Conditions/Physical Demand:

    While performing the duties of this job, the employee is frequently exposed to outdoor weather conditions, risk of exposure to pathogens and unsanitary conditions in client’s homes.
