Position Summary:

The LifeMoves CONNECT program follows families and individuals who successfully exit a LifeMoves program into stable housing, both through monthly text and email surveys, and light case management as needed. The CONNECT program serves as a means of assisting these individuals to remain stably housed, while also providing LifeMoves with much-needed data to continue informing and guiding LifeMoves programming. LifeMoves is seeking a highly motivated Case Manager to join our growing CONNECT program. Under the supervision of the Director of Shelter & Services, through monthly surveys and other periodic communications, CONNECT Case Managers follow individuals who successfully exit LifeMoves programs, with the ultimate goal of helping them maintain stable housing and self-sufficiency. In addition, CONNECT Case Managers are responsible for gathering, analyzing, and interpreting long-term outcomes data to inform and enhance future LifeMoves programming.

Primary Duties:

  • checking in with clients and providing light case management services such as risk assessment and community resource linkage
  • enrolling and following eligible clients following program exit
  • building, running, and interpreting reports on outcome data surrounding housing, safety, etc.
  • developing and facilitating training workshops for clients and LifeMoves staff
  • collaborating with external research partners to:
    • provide individual- and program-level data to inform LifeMoves programming
    • develop and implement outcomes evaluation projects
    • support development of research manuscripts for publication
Competencies:
  • Advocacy: Proactive communication about the needs of clients, and how LifeMoves can improve their service delivery to best support success, well-being and cultural sensitivity
  • Clinical Engagement: Ability to establish effective relationships with all clients regardless of their challenges and complexity
  • Resourcefulness: Possess the knowledge and ability to find and secure necessary support for clients by building relationships with community resources, which includes researching best practices in the field
  • Data Analysis: Possess the technical and analytical skills to read, analyze, and interpret data to inform LifeMoves programming
  • Organization: Ability to multi-task, be flexible, and track several work streams simultaneously while remaining responsive to client needs
Position Qualifications

  • Bachelor's degree in social work, psychology, or similar field AND minimum of two years' experience working with homeless individuals or other under-served populations in crisis OR Master's degree in social work, psychology, or similar field is preferred
  • Excellent communication and interpersonal skills; proven ability to collaborate with other agencies
  • An understanding of ideas and practices that promote diversity, equity and inclusion; and a commitment to ongoing professional development in this area.
  • Excellent computer skills and experience with data entry
  • Strong work ethic; comfortable working independently
  • Automobile and valid driver's license required
