OBJECTIVE: The Program Director assures activities and programming meet the individual and group needs of all participants in accordance with Adult Day Services philosophies, policies and procedures. The Program Director maintains contact with participants, families and caregivers to assure quality and continuum of care. The Program Director observes participants for social and physical needs, making recommendations and referrals as needed. The Program Director supervises and evaluates programs, staff, volunteers and conducts administrative duties.

ESSENTIAL FUNCTIONS:

Programming

1. Conducts monthly Participant Council meetings.
2. Develops and distributes monthly activity calendar in conjunction with Participant Council minutes.
3. Assures community outing sites are safe and handicap accessible.
4. Assures age appropriate programming, decorations and professional delivery of service, which reflect participant respect and dignity.
5. Implement the day-to-day activity/programming functions.
6. Coordinates transportation for community outings.
7. Utilize resource materials and community to enhance programming.
8. Observe and record participant involvement: daily attendance and/or progress notes.
9. Maintain inventory for daily snacks and recreational programming.
10. Develop and post monthly snack calendar.
11. Assure that exercise program, and its delivery, is conducted as approved by consulting Physical Therapist.

Participants

1. Document and report unusual signs, symptoms or personal changes to staff, RN, caregiver, social worker.
2. Develop individual programming objectives and goals (care plan) for each participant based on specific needs.
3. Review care plans quarterly.
4. Ensure individual care plan objectives and goals are being monitored.
5. Receives referrals and conducts home visits (as qualified).
6. Assist participants with personal and/or physical needs in conjunction with or in the absence of other ADS staff.
7. Assist with participant transfers, ADL's or treatments as needed/directed by Physician or care planning team.
8. Provide ongoing emotional support, educational information to participants and family. Facilitate referrals to community services as needed.
9. Complete discharge summary as appropriate.
10. Utilizes electronic systems of documentation as required per facility policy.

Administrative

1. Conducts Adult Day Services affairs.
2. Participates in the selection process for Adult Day Services staff and volunteers.
3. Document and reports incident and Vulnerable Adult reports as outlined in the Policy and Procedure Manual.
4. Supervise staff and volunteers providing input to the Executive Director to assure quality of services to participants and families.
5. Purchase snack and recreational inventory within budgeted guidelines.
6. Attends/participates in relevant, professional organizations, community agency boards and task forces.
7. Maintains current certification in first aid, CPR and other affiliations.

Marketing/Public Relations

1. Serves as a liaison, facilitator and resource for all entities in Adult Day Services and community at large.
2. Promotes Adult Day Services at local health fairs and appropriate community events.

Safety/Facility Maintenance

1. Follows the Volunteers of America National Services Abuse Protection Plan and reporting procedures as outlined.
2. Monitors the participants and staff in practicing good hand-washing and infection control techniques.
3. Maintain Adult Day Services sites in neat, clean and orderly fashion.
4. Report repairs needed to appropriate persons and makes minor repairs/adjustments as able.

Attends required in-services & completes assign on-line modules.

Job Requirements

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. 1. Licensure by the state in which facility is located as a licensed practical nurse or registered nurse; or completion of at least two (2) years of post-secondary education from an accredited college, university, technical college or correspondence school; and
2. Minimum of two (2) years experience planning or delivering health or social services, including supervision and administration.
3. Valid drivers license in state of employment.
4. Knowledgeable and responsive to the needs of older individuals.
5. Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

8. Ability to see and interpret colors as in reading warning lights.
9. Ability to hear normal sounds within background noise.

11. Ability to keep attention on task if routinely interrupted.
12. Ability to understand many ideas/approaches.
