Located at1100 First Avenue South in beautiful Sleepy Eye Minnesota, Sleepy Eye Area Home Health provides a variety of Home Care Services at Countryside Retirement Community, Ross Park Apartments and in the community of Sleepy Eye. Our goal is to help our clients to maintain independence and health intheirown homes.

We are currently seeking a RN to lead our team of health care providers!

The Director of Health Care Services provides direction, coordination, supervision and implementation of professional and supportive services to residents/clients using nursing theory and process in accordance with the Nurse Practice Act. The Director of Health Care Services assures the residents/clients are treated with respect and dignity, recognizing individual needs and encouraging independence.

Thisrole will be in a on call rotation with other nurses.

Core Responsibilities:

• Implements systems to assure compliance with all state and federal and local laws.
• Provides leadership to clinical and managerial staff.
• Assure compliance with the requirements of state licensure through policy and procedure development. Staff education and ongoing monitoring activities.
• Introduce health services to new clients and families.
• Perform initial and ongoing assessment of physical & psychosocial needs, coordinate with other departments, agencies and care providers as needed.
• Establishes Quality Improvement planning teams.
• Make health care staff hiring decisions.
• Participates in the hiring and orientation of personnel.
• Ensure staff development and in-service education programs to ensure competency of staff and to meet regulatory requirements.

QUALIFICATIONS:
  • Registered Nurse with minimum of two (2) years experience in nursing supervisory or nursing management in a long-term care setting or equivalent preferred.
  • Demonstrates knowledge of gerontology, rehabilitative, and medical-surgical nursing care, Antibiotic Stewardship, Infection Prevention and Infection Control.
  • Knowledge of State and Federal regulatory requirements for long-term care.
  • Understanding of current Infection Prevention and Control and OSHA guidelines.
  • Knowledge of QAA/QAPI theory and process.
