Case Managers provide treatment services to families including skills training and education designed to enable families to care for their mentally ill or emotionally disturbed children at home. Maintains regular communication with families, schools, physicians, and other service providers involved with the children and family. Works flexible hours, including evenings and weekends, to meet the needs of children and families on caseload. Works with FBMHS team members to develop a comprehensive treatment plan for child and family according to policies. Travels to client's homes, other agencies, and community resources for purpose of arranging and providing community and home-based services for children, adolescents, and families. Provides planning, family support, service linkage, and referral for families and children on caseload. Works closely with other FBMHS team members to assure clear communication and team consistency. Completes necessary documentation of each case, which complies with the standards set by Merakey, the county MH/MR, OMHSAS, and other licensing/regulatory organizations. Adheres to all Merakey, County, State, and Federal policies and regulations related to mental health treatment services. Complies with Merakey ethics and code of conduct, in all contact with clients, co-workers, and community. Participates in regular clinical supervision and continuing education activities to further develop professional skills and to meet FBMHS requirements. Participates in Merakey quality improvement/quality assurance activities.

Earn $17.70/hour.

POSITION REQUIREMENTS

• Bachelor's degree in psychology, social work, or related discipline.

• One year of experience with children and adolescents

• Must possess valid PA Driver's License and access to reliable transportation
