Responsibilities

The Patient Screener will screen patients for unmet social needs, deliver a customized list of community resources, conduct patient interviews and develop action plans to address identified needs, and conduct follow up call to patients and community service providers. This is an entry level position that will support the Community Connection Project and administrative staff. This position will also support community outreach activities and events.

Qualifications

• High School Diploma or GED

Experience

• Relevant Experience

Certification and Licensure

• PA Drivers License
• Certified Medical Interpreter License a plus
