Support Services for Veterans Families (SSVF)

Location: Dallas, TX

Job Type: Full Time

Pay: $55,000 per year

Day-To-Day

• Acts as a liaison between the SSVF program and the VA or community medical clinic and other healthcare providers
• Coordinates care for a population of homeless veterans with complex needs who require assistance accessing health care services.
• Works closely with the veteran's case management personnel.
• Works with SSVF program team to provide timely, appropriate, veteran centered care in an equitable manner.
• Conducts non-clinical assessments of the veteran to understands the veteran's situation, potential barriers to care and impact of such barriers on the veteran's ability to access and maintain health care services.
• Works closely with veterans to assist them in communication their preferences in care and personal health-related goals to facilitate informed/shared decision making for care.

Qualifications

• Bachelors in Human Services or related field
  
• 1 year experience working with people who are homeless, veterans and/or disabled
• Any combination of 1 years health/social services experience and/or education
  
• Valid Texas Driver's License and ability to be insured.
