Summary of Duties

The Home Care RN provides, coordinates, teaches, or supervises nursing care for a group of patients usually in the setting of the patients' home. Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the nurse provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Home Care RN ensures adequate service provision, including scheduling of visits, based on patient's identified needs and priorities for care. Complies with all Home Care policies and procedures.

Essential Job Functions

• Performs comprehensive assessment of patient
• Establishes a treatment plan that reflects the patient's diagnosis and goals as addressed in the assessment and physician orders
• Adheres to the specified frequency and duration of visits ordered by the physician
• Provides patient education
• Works collaboratively with a multi-disciplinary team to provide the best possible outcome for our patients

Experience

Two years nursing experience required. Prefer two years of experience in home care, critical care, oncology or emergency care.

Requirements

Current DC RN licensure and willingness to become licensed in MD

Current CPR certification

Reliable transportation
