POSITION SUMMARY:

Responsibility includes the provision of clinical services and the coordination of care for transplant and living donor patients. Provides education and clinical support for staff in the Transplant Center and in the hospital. Works cooperatively and effectively as a member of the Multidisciplinary Care team.
POPULATION SERVED:
All employees, staff of LPMC in both clinical and non-clinical departments, including patient/families, physicians, volunteers, agency, contract, students and the community served.
ESSENTIAL FUNCTIONS:
  • Ensures the coordination of the clinical aspects of the transplant candidate and potential donor.
  • Participates in Transplant Education Class for patients.
  • Monitor transplant patients and living donor medical and psychosocial status.
  • Coordinates all surgical arrangements for living related donor transplants.
  • Prioritizes data collection according to the patient's condition or needs.
  • Provides data coordinator with all information necessary to meet UNOS and Medicare regulations.
  • Participates in Transplant Education Class.
  • Participates in Annual Transplant Hospital Staff Orientation Program.
  • Fosters cohesiveness and unity among the Multidisciplinary Care Team.
  • Maintains open communication between the Multidisciplinary Care Team to identify challenges and resolve issues and address process improvement opportunities in a team oriented approach.
  • Participates in multidisciplinary care team planning and collaboration including selection committee.
  • Initiates and/or participates in staff education such as orientation, development of educational materials, team building, and in-service programs.
  • Applies knowledge of the principles of growth and development over the life span to identify patient and family requirements relative to age-specific needs.
  • Assess potential learning problems (language barrier, readiness to learn, etc.) and structures teaching sessions to meet individual needs.
  • Act as a liaison between the transplant center and dialysis facilities.
  • Communicates with Transplant surgeon/Transplant physician and referring centers and physicians in a timely manner.
  • Rotates 24-hour call duty for donor screening, coordination of transplant team for decease donor transplants, and triage of patient calls.
  • UNet Database documentation.
  • Participates in transplant and organ donation awareness initiatives Keeps abreast of research/literature/developments in nephrology and transplant and attends appropriate continuing education institutes and conferences.

  • Maintains a working knowledge of regulatory agency policies and procedures to ensure that transplant services are provided in accordance with facility/customer needs and performed in accordance with UNOS, CMS, JCAHO and Las Palmas Medical Center policy and procedure.

  • Other duties as assigned.

Qualifications

REQUIRED LICENSURE/CERTIFICATIONS
  • Current licensure in Texas as a Registered Nurse
  • BCLS
REQUIRED EDUCATION/EXPERIENCE
  • 1 year critical care nursing experience; experience with renal transplantation or dialysis preferred.
  • 2-3 years clinical nursing experience or 1 year transplant nursing experience.
PREFERRED EDUCATION, LICENSURE, CERTIFICATION, EXPERIENCE, SKILLS, KNOWLEDGE AND ABILITIES
  • Bachelor degree in Nursing from an accredited institution preferred
  • Transplant Coordinator experience
  • Certified Clinical Transplant Coordinator preferred
