With the support of the community, the Dementia Care and Education Campus, an innovation of Hospice of the Valley, is a comprehensive resource for those living with all stages of Alzheimer's disease and other forms of dementia. The Campus will provide full-spectrum care for persons living with mild, moderate, or advanced dementia and support for their families. A 12-person Assisted Living Center, a 24-member Adult Day Club with adjoining Child Center, and a 12-person Hospice Inpatient Care Home will provide superb and creative person-centered care. The Community Education Center will educate and inspire physicians, nurses, social workers, caregivers, occupational therapists, music therapists, families, and the community to improve quality of life for persons living with dementia.

Position Profile

The Dementia Program Nurse Practitioner provides expert palliative dementia care resource support and promotes HOV's leadership role in dementia care. Promotes patient/family/caregiver-centered care through exemplary customer service and utilizing pharmacological and non-pharmacological therapies in support of comfort and quality of life for supportive care and hospice patients. Provides education within the agency and in the community to promote excellence in dementia care.

Responsibilities

  • Provides expert palliative dementia care resource support.
  • Promotes HOV's leadership role in palliative dementia care.
  • Develops and maintains therapeutic relationships.
  • Plans care and intervenes effectively.
  • Provides effective dementia and delirium assessment and management.
  • Promotes patient safety.
  • Facilitates smooth transition between care environments
  • Provides effective patient/family/caregiver teaching.
  • Manages end-of-life ethical issues effectively.
  • Collaborates in providing patient care.
  • Creates timely and accurate documentation.
  • Utilizes resources effectively and efficiently.
  • Adheres to scope of practice/HOV standards and facilitates continuously improved processes/services.
  • Maintains and enhances professional skills.
  • Adheres to high standards of personal and professional conduct.

Minimum Qualifications

Education
  • Graduate of accredited Nurse Practitioner program and board-certified.

Work Experience

  • Minimum of one year working with persons with dementia.
  • Minimum of one year of recent experience in medical, surgical or critical care nursing.
  • Experience in the psychological, psychosocial and medical aspects of terminally ill patients.

License/Certification

  • Current Nurse Practitioner license in good standing
  • Current CPR certification
  • Valid Arizona driver's license and insured automobile.

Knowledge

  • Knowledge of and ability to apply nursing process.
  • Knowledge of self-care, personal boundaries; abilities, limits and inner resources.

Skills/Abilities
  • Excellent verbal and written communication skills. Ability to communicate complex and sensitive information.
  • Highly developed interpersonal skills including active listening, questioning and observation skills; ability to meet people with ease, develop positive rapport and demonstrate empathy, acceptance, sensitivity, humility, diplomacy, presence and confidence. Ability to adjust approach based on patient/family/caregiver needs/goals.
  • Possess professional image, enthusiasm and self-motivation.
  • Ability to effectively collaborate with interdisciplinary team in providing patient care.
  • Ability to adapt to unpredictable situations in patient care setting.
  • Strong clinical assessment skills; ability to accurately review body systems, identify problem areas and assess needs for appropriate interventions.
  • Excellent critical thinking/problem-solving skills. Ability to function independently and effectively anticipate needs and problems. Ability to recognize capabilities and utilize others/resources to optimize patient care. Ability to effectively delegate while maintaining oversight.
  • Ability to accurately calculate mathematical figures related to medication and dosages.
  • Skilled in organizing and prioritizing work and manage multiple tasks and assignments
  • Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through with minimal supervision.
  • Ability to deal with confidential information in a professional manner.
  • Proficient computer skills.
