Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

The Program Coordinator-Housing Programs is responsible for overseeing all housing services for formerly homeless individuals, including the Permanent Supportive Housing Continuum of Care (COC) and other housing grant programs. This position works collaboratively with the City of Indianapolis on COC funding, the Eskenazi Grants Department and the COC, including serving on various committees for the COC, all collaborative agencies involved with housing programs to include Coalition for Homeless Intervention and Prevention (CHIP) and various city-wide agencies.

Essential Functions and Responsibilities

  • Responsible for all housing programs and provision of housing services to formerly homeless persons, to include oversite and supervision of COC PSH grants, and direct supervision of PSH staff
  • Works in collaboration with Eskenazi Grants Department to oversee SAMHSA/CABHI grant, including oversite of collaborative agencies, direct supervision of SEMHC staff involved in the program, iHOST Steering Committee and Eskenazi Grants Department
  • Provides administrative supervision to Clinicians, Care Coordinators, Community Support Specialists and Team Leads
  • Provides orientation and on-the-job training to assure staff is well prepared for their roles
  • Collaborates/consults with Psychiatrists, Clinic Managers and other personnel for the integration and development of necessary treatment services
  • Provides consultation and education to community groups and the public
  • Works collaboratively with other organizations to assure continuity of services for our clients, developing collaborative systems of care and referral processes
  • Monitors work performance, prepares personnel data and performance evaluations, as assigned
  • Assists staff in preparing documentation, including letters to court, evaluations and professional correspondence
  • Assists the Grants Department as required managing contracts, grants and other service agreements to assure outcomes required are met, and budgets are on target in a fiscally responsible manner
  • Works in coordination with with Eskenazi's Grants Department to ensure grant and contract reporting is completed accurately and in a timely manner
  • Evaluates need for program changes and proposes changes to Grants Department and administrative staff
  • Alters activities/behaviors to reflect and ensure adequate care appropriate to the age of the clients
  • Fulfills duties noted in area specific competencies (Adult, Acute, Youth or Addictions services)
  • Assists in developing and implementing/monitoring policies and procedures
  • Participates in program planning, development and evaluation
  • Assists in development of program budgets; oversees staff productivity
  • Acts as an advocate for clients within the COC, with landlords, and within the courts
  • Remains knowledgeable about the COC as a whole, and about trends or directions by sitting on various COC committees
  • Acts as an advocate in the community for the needs of our clients, organizing community groups to respond to those needs, when appropriate
  • Participates in City of Indianapolis, COC and HUD audits
  • Participates in COC mandatory meetings

Job Requirements

  • Master's degree in human services field, plus five years of job-related experience
  • Valid Indiana driver's license that meets Eskenazi Health's driving policy requirements

  • Advanced knowledge of psychotropic medication, their uses and side effects

  • Ability to use communication appropriate to the age of the audience; age-appropriate cognitive abilities, skills, techniques and communications
