The Program Director is responsible for providing direct and indirect services to clients residing in Residential Placements. This is accomplished by using an assertive case management outreach approach based on the Rehabilitation Model. Services include housing acquisition, intensive case management, psychiatric rehabilitation, supportive counseling, crisis intervention, resource acquisition, service coordination and development of a natural support system. The Program Director supervises and coordinates with staff on the team to ensure that clients receive direct services frequently during the week and uses creative problem-solving skills to engage clients to work towards identified goals. The Program Director oversees referrals and admission to the program and coordinates placement and communication with all treatment team members.

The Program Director serves as the supervising Qualified Mental Health Professional (QMHP) for the Intensive Residential Treatment Services Program and provides general oversight and coordination of IRTS services to clients residing in a supervised residential program for individuals with chronic mental health conditions that impact their daily functioning. The QMHP coordinates referral and admission into the program and assesses appropriate placement based upon a review of client needs. The Program Director facilitates a weekly staff meeting with treatment team to discuss client's progress, needs and interventions. The Program Director is responsible for updating the client's treatment plan and providing supervision to a team of staff to ensure that IRTS services are provided to all enrolled clients on a daily basis as well as ensuring that all DMH program requirements are met by completing quarterly compliance audits. The Program Director is responsible for general supervision of staff and ensuring that the program is adequately staffed. The Program Director provides on call support to the program for crisis situations as needed.

This is a full-time, benefit eligible position. As a Supervisor this candidate will provide on-site training and observation of weekday/weekend day and evening shifts. Candidate will cover occasional weekend/evening shifts as needed.


ESSENTIAL FUNCTIONS - JOB SPECIFIC

* Assesses and monitors a client's adjustment to community living
* Monitors client participation and progress in organized treatment programs to assure the planned provision of services according to the client's individual treatment plan
* Participates in the development or revision of a specific individualized treatment plan including completion of the DLA-20 as well as timely and thorough communication to assessor regarding potential goals and objectives
* Provides individual assistance to clients in accessing needed mental health services including accompanying clients to appointments to address medical or other health needs
* Coordinates monthly psychiatry appointments within SNF or ICF and more often, if necessary
* Completes weekly treatment team meetings with IRTS staff
* Communicates with CPRC Clinical Director, Program Directors and other staff as needed regarding client status, progress and needs
* Assesses client's level of risks, strengths, and weaknesses and interviews clients, reviews records, and confers with other professionals to evaluate clients' preparedness for independent living or transition to other levels of care
* Encourages medication compliance and provides education regarding medications and monitoring of side effects
* Assists the client to access and utilize a variety of community agencies and resources to provide ongoing social, educational, vocational, and recreational supports and activities
* Intercedes on behalf of individual clients within the community-at-large to assist the client in achieving and maintaining their community adjustment
* Maintains contact with clients who are hospitalized and participates in and facilitates discharge planning
* Trains, coaches, and supports in daily living skills, including housekeeping, cooking, personal grooming, accessing transportation, keeping a budget, paying bills and maintaining living space(s)
* Develops monthly work schedule for IRTS staff and fills in as needed
* Monitors IRTS program for DMH program compliance
* Coordinates referral and admission process for referred clients
* Supervises Intensive Residential Support Specialists
* Assists in creating personal support systems that include work with family members, legal guardians or significant others regarding the needs and abilities of a client
* Encourages and promotes recovery efforts, client independence/self-care and responsibility
* Provides support to families in the area such as treatment planning, dissemination of information, linking to services and parent guidance
* Provides holistic, person-centered care
* Develops and supports wellness and recovery goals in collaboration with the individual served
* Provides services to clients which are determined to be appropriate based on DLA score and assessed level of functioning
* Performs job duties with an efficient use of time based on client's needs as well as meeting efficiency requirements based on budget parameters
* Notifies Supervisor of significant occurrences related to clients, facilities or service needs
* Provides back up to staff during/after emergency calls and situations.
* Although driving is not an essential function of the position, a valid license and agency established minimum automobile coverage are required in the event the employee drives during employment.

SUPERVISORY RESPONSIBILITY

* Oversees day-to-day operations and tasks of supervisees
* Completes performance development/AIM process
* Time card management - manages time sheets
* Ensures progress and development is made toward departmental strategic plan goals



EDUCATION

* Master's degree in psychology, social work, counseling or related social service field from an accredited college or university
* Must be a Qualified Mental Health Professional (QMHP)

WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS

* At least two (2) years of experience in mental health field and/or experience working with adults with chronic mental illnesses required
* Experience working with individuals residing in residential care settings preferred
* Supervisory and program development experience desired
* Must have an automobile, a valid Missouri Class E driver's license with a clean driving record and current automobile insurance with required coverage limits

LICENSURE/CERTIFICATION: Must obtain and maintain certification in CPR, First Aid, CPI and Medication Level I Aide
