Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Centeroffers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Job Role Summary
The Resource Coordinator uses intake information provided by the resident to develop a resident-centered care plan. The Resource Coordinator arranges services for residents residing in the
Assessment and Intervention Center and seeks to connect them to appropriate community providers. The
Resource Coordinator may conduct assessments with a focus on resident empowerment.
Essential Functions and Responsibilities
• Proactively contributes to Eskenazi's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County. Models Eskenazi values.
• Endorses and models a strengths based, community centered model of care.
• Coordinates referrals up to 60 Residents who are in the Assessment and Intervention Center
(AIC), with a key focus on resident empowerment and assuring that Resident referrals and coordinated services are based on resident choice.
• Reviews assessments, service plans, progress notes and other records and documentation for accuracy; schedules daily tasks for intake interviews and individual assessments.
• Develops resident service and discharge plans collaboratively with residents and other AIC Staff, to include medical and management staff.
• Works with clients in developing skills for community integration and resource acquisition both in individual and group settings.
• Confirms proposed resident placement with treatment providers; acts as a liaison in the "warm handoff"' to treatment providers based on the care plan; may include arranging visits and transportation.
• Coordinates resident placement in other needed services; may include working with psychiatric, medical, dental, job training, workshops and other identified community services as needed; responsible for periodic monitoring of resident interaction with community provider post-discharge.
• Maintains positive relationships in a professional manner based on the mission of the AIC and with the best interest of the resident in mind.
• Responsible for data collection and data entry including coordination with information from community partners; completes and prepares reports as requested.
• Provides input on an ongoing basis to develop and improve program servicing and delivery
• Keeps current by reading memos, directives, meeting minutes and revised policies and procedures as posted.
• Participates in education and training as required for the position including but not limited to any continuing education and basic medical (CPR, basic First Aid, etc.)
• Attends required department meetings
Job Requirements
• Associate's degree in a relevant field and one year of experience working in behavioral health and/or substance use disorder treatment; three years of experience working in behavioral health and/or substance use disorder treatment and a state recognized certification may be substituted for the degree
• Valid Indiana driver's license and current automobile insurance policy as required by the state of
Indiana
• Certification as a trained Assessor/Counselor/Case Manager for chemical dependency is preferred
Knowledge, Skills & Abilities
• Knowledge
• Experience with and personal understanding of homeless, alcohol/substance abusing persons, dual diagnosed, criminal justice, veteran, and/or culturally diverse population
• Knowledge of community treatment partners and the city of Indianapolis' Continuum of Care
• Skills
• Skill with use standard office equipment including computers, telephone/fax, copier/printer and scanner; proficiency in Microsoft Office applications
• Solid communication effectively in verbal and written formats
• Ability
• Ability to establish and set priorities, providing and responding to time sensitive information and requests as required
• Ability to plan and accomplish assignments in a timely manner with a high emphasis on placing information into the various data formats, and the individual personnel filing system
