The Opportunity: Surgical Care Coordinator-RN

Why This Role Is Important

The purpose of this position is to provide planning, coordination, integration, education and the ongoing evaluation of the delivery of care to the specialty, surgical population. The Surgical Care Coordinator is responsible for assuring the delivery of integrated, specialty care in the periprocedural and perioperative settings. The Surgical Care Coordinator facilitates a seamless transition of care throughout the continuum in collaboration with patients, physicians, vendors and all stakeholders involved in the care of the patient. The Surgical Care Coordinator drives care coordination by actively providing consultative screening and evaluation of patients and improve patient, family, and physician satisfaction. The Surgical Care Coordinator serves as a link between a patient, their family, the physician, and the care team caring for them.

This position exists to assure and evaluate delivery of evidence-based care according to national and statewide clinical practice guidelines; assist in developing clinical pathways; and assuring care management occurs in a cost-effective manner while enhancing patient safety and the overall customer experience.

What You'll Work On

Responsibilities:
  • Serves as a resource for all health care providers in the patient care coordination and management of outcomes.
  • Conducts patient and family pre-admission teaching to facilitate adherence to the care plan and expectations; promote a positive outcome and develop patient independence through postoperative self-care.
  • Assists in the coordination of any necessary pre-surgical testing and post-surgical medical equipment.
  • Coordinates with the patient care team during the patient's stay to enhance patient satisfaction and ensure quality care is being delivered to meet specific discharge criteria and goals and coordinates any post-op care including but not limited to PT, OR, and Case Management for home care.
  • Becomes a point of contact for questions throughout the entire outpatient process.
  • Accountable for maintaining patient education materials to educate patients and families.
  • Collaborates with all aspects of the healthcare team through participation in program team meetings to ensure a quality-of-care delivery process for optimal patient care.
  • Participates in the development, implementation, and evaluation of standards of care, patient management tools (pathways, protocols), clinical practice guidelines, policies and procedures and data collection and interpretation for continuous quality improvement.
  • Utilizes principles of performance improvement process to improve patient care.
  • Maintains a flexible schedule as necessary to meet the needs of patients, staff and system.
  • Uses innovative techniques to effectively manage resources to positively impact patient outcomes in a cost-effective manner.
  • Ensures compliance with quality measure initiatives appropriate to the service line.


Qualifications
Experience
  • Required:
    • Minimum of three (3) year in Ambulatory Surgery, Outpatient Surgery, Recovery Room, or Medical/Surgical Nursing.

Education/Degree
  • Required:
    • Completion of a minimum of three (3) years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required.
    • Bachelor's Degree in Nursing is preferred.
Licensure, Certifications, or Registration
  • Required:
    • Currently holds American Heart Association (AHA) Basic Life Support for Healthcare Provider (BLS) or approved equivalent.
    • Currently holds a nursing license in the state of New Hampshire, free of sanctions and disciplinary actions.
