General Responsibilities

In collaboration with Care Coordination staff, this position is directly involved in the process of assessment, planning, facilitation, and advocacy for options and services to meet individual patient's health care needs. Through communication and identification of available resources, participates in the promotion of quality outcomes, patient satisfaction, and cost-effective care delivery. This position reports to the Manager of Care Coordination.
Schedule: This position has weekend rotations.

Basic Qualifications

Education and Training: Master's degree in Social Work required.

Certification and Licensure: Licensed Social Worker (LSW) in the State of Pennsylvania a plus

Skills:
• Community resources
• Entitlements
• Managed care, government payers, third-party reimbursement
• Information systems, Midas preferred
• Matching patient and family with community resources
• Using MS Office applications
• Developing and maintaining strong interpersonal relationships

Experience: Three (3) years of experience in a hospital, home health, managed care environment, or other health related services/settings.
