Job Summary:
Assists in the development, implementation and maintenance of wellness and/or exercise programs for patients, employees and/or the community. Provides coverage in the field ensuring that screenings are completed accurately, safely and on time. Works with team members, health system and employer group leaders/members and patients to provide appropriate resources to meet health care and wellness needs focusing on all dimensions of wellness, connection to appropriate resources, overcoming barriers and capitalizing on strengths. Develops and oversees wellness programs, promoting to customers as appropriate. May require ability to travel to customer locations and work varied schedule.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Develops, promotes, delivers, evaluates and grows health and wellness programming to meet the interests, needs, and well-being of program participants.
  • Presents learning and educational opportunities assisting with resource navigation for program participants.
  • Works collaboratively with other team members to support programming for both internal and external events such as health fairs, health screenings, seminars, classes, wellness rounds, etc.
  • Performs health risk appraisal screening such as bio-metric screening including BMI, body composition, risk factors, barriers and creating personal wellness plans involving all dimensions of well-being.
  • Documentation and reports and dashboard creation to demonstrate health improvements and program participant health engagement outcomes.
  • Collaborates with internal and external partners in evaluating innovative health and wellness programs and solutions.
  • Develops marketing information to promote healthy lifestyles and engagement in available resources.
  • Provides support to program participants through effective education via protocols and working collaboratively with the multidisciplinary team within and outside of the organization.
  • Performs other duties as assigned.

EDUCATION
  • Bachelor's degree in health promotion, health education or related field, or equivalent years of experience and education

EXPERIENCE
  • One year of healthcare or wellness related experience

PHYSICAL REQUIREMENTS
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Rare climbing.

Licenses / Certifications:

Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA)
