Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Centeroffers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Focus of Child & Adolescent Wraparound Facilitation

The Child and Adolescent Wraparound Facilitation (WRAP) program is a collaborative effort designed to identify and engage youth who experience serious mental health or behavioral health challenges who meet specific eligibility criteria. The wraparound process is led by a wraparound facilitator who will help the child/youth build a team of the people that provide support. The support network may include friends, family, therapists, faith-based support and other caseworkers and services that the family are receiving. The goal is to have one plan that is created with the team that is created and is shared with everyone so those who are supporting the family can work together on the goals. The program is designed to support the strengths and challenges of each family with a focus on empowerment, resiliency and hope.

Where you Fit In:

Wraparound Facilitator partners with the families to develop their multi-perspective family story and pulls from that story a team who then develops a strength-based plan of care with strategies to best support the family's needs. This role serves as a point of contact to correspond and coordinate partnerships to align with the plan of care for every family that is assigned. Throughout the wraparound process, the Wraparound Facilitator brings teams together monthly to measure progress and to update the plans of care submitting to them to the Division of Mental Health and Addiction for approval. The Wraparound Facilitator manages the money and hours of all wraparound providers invested in the plan of care. The Wraparound Facilitators submit wraparound hours based off strategies in the plan of care and this creates the notice of action allowing all wrapround providers to be paid for their work. Wraparound Facilitators will also review provider's monthly reports to ensure they are working on goals developed during the wraparound team meetings and within the Division of Mental Health and Addiction guidelines. Wraparound Facilitators will also complete assessments throughout the year in which families are enrolled in wraparound to monitor progress and to guide the team's decision-making process.

The wraparound team members are very collaborative and supportive of one another. The team works together on developing strength-based solutions that can be identified within the family's community and accessed after the youth has graduated from wraparound at the end of the year. Wraparound solutions are often creative and utilize out of the box thinking in effort to ensure the youth we serve remain in the home.

What to Expect:

Our Wraparound Facilitator provides at least 50% of their services in the community, which they create their own schedule for, providing flexibility each day on where they travel. Some evenings are required as both youth and caregivers need to be present at a wrap team meeting.

Am I Qualified?

  • Bachelor's degree in human services, psychology, or education,with two or more years of experience in clinical, case management, skills building, child welfare, juvenile justice, and/or education in a K-12 school setting.
  • Must possess or obtain wraparound DMHA Certification (will obtain during employment)
  • Valid Indiana driver's license that meets Eskenazi Health's driving policy standards; valid vehicle registration and insurance must be obtained.
  • Clear Department of Child Services background check.

Essential Functions and Responsibilities

  • Performs assessment of youth/family needs through the development of multi-perspective family story/timeline
  • Guides the family through the wraparound process.
  • Implements the four phases of wraparound; Engagement & team preparation; Initial plan development; plan implementation and transition.
  • Builds and develops child and family teams, facilitating monthly child and family team meetings and additional crisis meetings, as necessary.
  • Maintain necessary documentation regarding meetings and progress of the care plan.
  • Completes care plan, crisis plan, intervention plan, plan service authorization, minimum data set, eligibility reviews, and participant status with the designated system, TOBI.
  • Uploads all Monthly Child and Family Treatment Team documentation within TOBI system for DMHA review.
  • Uploads all Monthly Child and Family Treatment Team documentation within EPIC system for SEMHC coders review.
  • Review and incorporate DMHA Supervisor feedback to the team in an effort to increase the wraparound fidelity to the model.
  • Advocates for youth and their families through community agencies while adhering to the Wraparound principles.
  • Complete regular assessments utilizing the CANS Assessment tool and corresponding documents.
  • Develops comprehensive transition plans with discussion occurring in the team development phase to ensure adequate natural supports/family voice for long term success.
  • Maintains updated treatment plans with progress notes in the timeliness per agency guidelines.
  • Completes monthly client check within Indiana Medicaid Portal to verify program eligibility and status.
